                   Video script

                     1. Doctor    Now, Mr. Smith, we have the results of your gastroscopy.  As we
                                  suspected, there are some signs of erosion in your esophagus.  So you will
                                  need to continue the tablets I gave you last time.  You’re also going to
                                  need to make some changes in your lifestyle.  First of all, do you drink
                                  coffee or tea?
                     2. Patient    I have a couple of cups of coffee first thing in the morning.  And I usually
                                  have a cup about mid-morning.
                     3. Doctor    Well, caffeine makes the acid build up in your stomach and aggravates the
                                  problem.  You really should try to cut out all caffeine

                     4. Patient   But, I usually…
                     5. Doctor    I know it’s difficult. Can you cut out the mid-morning coffee? And try to
                                  eliminate one of your two early-morning coffees?
                     6. Patient   Well, the mid-morning coffee shouldn’t be a problem. But I don’t know
                                  about the morning.
                     7. Doctor    Try. The more you can cut down, the better. Then,  do you smoke?

                     8. Patient   Yeah, half a pack per day.

                     9. Doctor    You know that smoking is dangerous for a number of reasons. …but
                                  especially in your case. It also increases the acid in the stomach. You’ll
                                  really have to try to quit. Have you ever tried?
                     10.Patient   About three years ago, I stopped smoking for one year, but then I went
                                  through a difficult time and started smoking again.

                     11.Doctor    Well, there. You know you can do it. Why don’t you try again? If you
                                  have any trouble, we can discuss using a patch or something like that, but
                                  it’s better to do it on your own, if you can. Another thing you are going to
                                  need to do is to sleep with your head elevated. That will keep the acid
                                  from rising up in the esophagus. Can you raise the head of your bed? Or
                                  use two or three pillows to sleep.
