Life Insurance
Employee Life Insurance
Life insurance pays a benefit to your beneficiary in the event of your
death. EFH provides you with basic life insurance equal to one times your
annual base pay at no cost to you. You may choose to purchase additional
coverage of up to six times your annual base pay (rounded up to the next
$1,000), up to $2 million in optional coverage.
If you didn’t enroll in optional coverage when you were first eligible and
choose to do so at a later date, you will be required to provide evidence of
insurability (EOI). Additionally, you must provide EOI if you choose coverage
of four, five or six times your annual base pay or if you choose to increase
your coverage by more than one times your annual base pay during open
enrollment.
Your premiums for optional employee life insurance are based on your age,
annual base pay and whether or not you use tobacco products. You are
considered to “use” tobacco products if, in the previous year, you smoked
cigarettes, pipes or cigars, used snuff or chewed tobacco. You must notify
EFH if you have a change in tobacco user status.
Your salary and age will be frozen as of October 1, 2016 (or your date of hire
if hired after October 1, 2016). Premiums and benefits paid in the event of
your death will remain the same throughout 2017.
