Bonnyton House Care Home 3

        Moving in to Bonnyton House

An appointment will be made to visit you by the Manager or another member of Senior Care Staff to carry
        out a pre-admission assessment

The purpose of this visit is to obtain details about your medical, emotional and social needs. The assessment
will include details about your mental health. This assists the staff to provide the best care to you on
admission. Based on the results of the assessment a decision will be made regarding whether the Home is
        able to meet your needs.

The information gained will be used to help us formulate your Individual Care Plan.

If the person making the assessment feels that the Home is able to meet your requirements you will be
invited to visit the Home if you have not already done so. If this is not possible a relative or friend may be
able to visit on your behalf. During the visit you will meet Staff who will work with you, be shown your
bedroom, be shown facilities of the Home and have further opportunity to ask questions you may have.

On the day of your admission you can be sure of a warm welcome. You will be taken to your room or
lounge (whichever you prefer) offered a cup of tea and be able to rest if you wish. You must always feel free
to ask questions at any time. You will be offered help to unpack or the Staff will do this for you. Your
medication will be checked and put into a secure place.

You are encouraged to bring personal belongings e.g. Photographs, ornaments, framed pictures, small items
of furniture, television or radio. If you wish larger items of furniture please discuss this with the Manager.
Due to fire regulations, building standards and Care Commission requirements certain items may not be
        permitted.

All the rooms including the bathrooms, toilets and public rooms have a nurse-call system, which you will be
        shown how to use.

You will be appointed a keyworker. A keyworker is a named member of staff who will help put together
your Individual Care Plan with you. It is that person’s duty to check progress with you and keep in touch
with everybody involved with your care. If you wish you will be given a copy of your Personal Plan.

You can rest assured you will be looked after by friendly caring staff who will encourage your independence
for as long as possible. They will provide all the help you require and try to make Bonnyton House as near
        to home as possible.
