5.5 Separation of Employment
In all cases of voluntary resignation (one initiated by the employee), employees are asked
to provide a written notice to their supervisors at least two weeks in advance of the last
day of work. The two weeks’ notice should be 10 actual working days. Holidays and paid
time off (PTO) will not be counted toward the notice. Human Resources will conduct an
exit interview on or before the last day of employment, including to collect all Company
property, and employees must cooperate fully with the interview. If applicable, information
regarding benefits continuation through the Consolidated Omnibus Budget Reconciliation
Act (COBRA) will be sent to the employee’s home address. You will be paid your final
wages, accrued and unused Paid Time Off (PTO) (if applicable), and if applicable, notified
of any cancellation in benefits, in accordance with applicable law in the office in which
you are located.
Should it become necessary because of business conditions to reduce the number of
employees or work hours, this will be done at the discretion of the Company, in
accordance with applicable law. For all separations, Company-owned property, including
vehicles, keys, identification badges, computers, credit cards and all confidential
information, must be returned immediately upon termination of employment, unless
otherwise communicated.
Verifications of employment are completed by Human Resources; they will verify name,
title, dates of employment only. _________
Retirement
Employees who wish to retire are requested to notify their Supervisor and the Human Resource
department in writing at least one (1) month before the planned retirement date.

5Benefits and Separation
Health insurance terminates the last day of the month of employment. Any employee
contributions toward their health insurance premium through the end of the month may be
deducted from the employee’s last paycheck. Information for Consolidated Omnibus Budget
Reconciliation (COBRA) continued health coverage will be provided. Employees electing COBRA
coverage will be required to pay the full monthly health insurance premium following separation
from the Company.
5.5.5 Exit Interview and Out-Processing
The separating employee will contact the Human Resource department as soon as notice is given
to schedule an exit interview and out-processing. The out-processing will be on the employee’s
last day of work or another day, as mutually agreed upon. In an effort to evaluate our programs
and policies, you will be given an opportunity to participate in an Exit Interview with Human
Resources or another management employee. Should you choose to forego the Exit Interview
