While educational assistance is expected to enhance employees’ performance and
professional abilities, XYZ Corporation cannot guarantee that participation in formal
education will entitle the employee to automatic advancement, a different job assignment,
or pay increases.
4.11 Health Insurance
XYZ Corporation health insurance plan provides employees and their dependents access
to medical insurance benefits. Employees in the following employment classifications are
eligible to participate in the health insurance plan:
• Regular full-time employees
Eligible employees may participate in the health insurance plan subject to all terms and
conditions of the agreement between XYZ Corporation and the insurance carrier.
Details of the health insurance plan are described in the [NAME OF YOUR GROUP
INSURANCE COMPANY] provided at the end of this document. Information on cost of
coverage will be provided in advance of enrollment to eligible employees. Contact the

insurance benefits.
4.12 Life Insurance
Life insurance offers you and your family important financial protection. XYZ Corporation
provides a basic life insurance plan for eligible employees.
Accidental Death and Dismemberment (AD&D) insurance provides protection in cases of
serious injury or death resulting from an accident. AD&D insurance coverage is provided as
