National Photo & Video Release Form

This Photo & Video Release Form (“Release”) is entered into by the undersigned parent
or legal guardian (“Guardian”) of the minor child (“Talent”) named below, in favor of
Baby Kid Teen Media Group (“Media Group”).

1. Consent to Record

I hereby grant permission for Baby Kid Teen Media Group to photograph, film, and
record my child’s image, voice, and performance during auditions, rehearsals,
showcases, interviews, and related activities.

2. Media Usage Rights

I authorize Baby Kid Teen Media Group to use, edit, reproduce, publish, and distribute
these recordings in the following formats:

• Print publications (magazines, flyers, posters)

• Digital platforms (websites, apps, e-learning tools)

• Broadcast media (TV, radio, streaming services)

• Social media (Instagram, Facebook, TikTok, YouTube, Threads, etc.)

• Promotional campaigns, educational content, and public outreach

• Archival and legacy projects honoring youth and families

This release applies to all current and future media formats, including those not yet
developed.
