Talent Release Form ALL STATES
Celebrating Young Voices. Honoring Family Legacy.

This Parent/Guardian Talent Release Form (“Release”) is entered into by the
undersigned parent or legal guardian (“Guardian”) of the minor child named below
(“Talent”), in favor of Baby Kid Teen Media Group (“Media Group”).

1. Consent to Participate

I, the undersigned Guardian, hereby grant permission for my child to participate in
auditions, performances, interviews, photoshoots, video recordings, and other creative
activities organized by Baby Kid Teen Media Group.

2. Media Release

I authorize Baby Kid Teen Media Group to:

 Record, photograph, and film my child’s likeness, voice, and performance
 Use, edit, publish, and distribute such recordings in print, digital, broadcast, and
online formats
 Include my child’s image and voice in promotional materials, educational content,
and public campaigns

This release applies to all forms of media now known or later developed, and may be
used for marketing, educational, or inspirational purposes.

3. Rights & Ownership

I understand that all recordings and materials created by Baby Kid Teen Media Group
are the exclusive property of the Media Group. I waive any right to inspect, approve, or
receive compensation for the use of such materials.
