o Provide the correct answer (this information will be utilized to verify the participant’s account should
they forget their login credentials)

Submitting the W9 Form:
To complete registration, participants must submit a W9 listing their full name and social security number. A link to the
W9 form is provided on the Easy Street registration page or it may be downloaded by clicking HERE.

To ensure correct documentation of the participant’s W9 form, a cover page is generated when downloading the W9
form, which includes certain required elements: Agency Name, Agency Code, Ownership, and Phone Number.
Participants must submit the completed cover page along with the completed W9 form.

Once the W9 and Cover Page are completed, they must be emailed to easystreet@kemper.com.
Business Tax ID numbers and Personal Identification Numbers are not allowed. Each account must go through Internal
Revenue Service (IRS) verification before any money may be redeemed. Easy Street points are not available for
redemption until the producer’s W9 information has been verified by the IRS. Upon verification from the IRS, the
participant will be notified by email of their account status.
