      This SOP's Purpose/Goal
      To perform an annual performance evaluation of employees.





      Standard Operation:
      1) Set evaluation goals and objectives
         a) Upon hiring, distributed expectations and goals for employee

      2) Set evaluation frequency
         a) Staff shall be evaluated annual (at minimum)
         b) Faculty shall be evaluated annually and after each class via a survey


      3) Prepare the performance evaluation form
         a) Supervisor shall complete the evaluation form
         b) Self-evaluation is required by I5O staff

      4) Schedule meeting to discuss evaluation


      5) Comments and signature of evaluation
         a) Discuss any improvements and expected goals and objectives for next annual cycle
         b) Sign evaluation (approved/disapproved or agree / disagree
         b) File evaluation
