Appendix C.1: Passports: Applications



                                                Subprocess










          The following items will be collected from the applicant (P01):





                  Completed DS-11 form
             1
                  Passport Photo (Can be taken in our office for an additional $10)
             2
                  Certified Birth Certificate or US Citizenship evidence that lists parents
             3
                  Photo ID – If applicant is a minor child, both parents must present their ID
             4
                  Photocopies of ID and Birth Certificate
             5                  DRAFT


                The passport photo and driver’s license photo must bear the likeness of the applicant or

         the ROD Passports staff must notify the US Department of State of any variance.  All applicants

         including children must physically appear before the agent. The ROD Passports staff will verify


         the applicant’s identification to confirm the identity of the applicant (P02). Once all items are

         collected, they are examined for completeness, accuracy, and legibility (P03).


                When the applicant has completed all required documentation, the applicant will provide

         the US Department State fees and the Guilford County Register of Deeds fees (P04). Cash or

         money order are the only forms of payment. The ROD Passports staff will cashier out the

         transaction in BIS and complete the identification portion of the application.  The applicant will

         verbally acknowledge the oath and sign the application (P05).



                The applicant is free to leave once they have signed the application.  They are given the

         contact information for the US Department of State for tracking and any future questions or

         issues.   Applicants can not follow up with the ROD office on the status of their application. The

         US Department of State will only discuss submitted applications with the applicant.                    44
