• When entering new orders, the Plan of Care Update must be entered before any Physician Order (e.g., Meds,
schedule, etc.)
o Click on New Order. Select Plan of Care Update. Add the Physician and other required order details.
o You will get a pop-up Warning about the need to complete the pathway that updates the POC. Click Update
POC.
o Select the Pathway, “Need for Oxygen Therapy and Skilled Teaching Regarding Oxygen Therapy”
 (Note: the system will make you scroll through other pathways not previously ordered. Click Yes or
No accordingly.
o Once the Pathway is ordered, change Order Type to “Physician Order” and in the Instruction box, write
“Need for Oxygen”. This will open the Add Medications option.
o Click “ADD” and enter Oxygen in the Search Routed Drug field. Click “Full” to search the full list. (You will
not find oxygen in the Quick list)
o Select the applicable oxygen. Select “Provider” for Financial Resp. Type in the liter flow in the Dose field.
Add the mode, i.e. nasal cannula or mask, in the Amount field. Enter dyspnea or hypoxia in the Reason
field. Enter start date.
o Click Save.

If Outside of a Visit
• Medical Records, then Features.
• Enter a “New Order” and free text in the Instructions box “Need for Oxygen Therapy and Skilled Teaching
Regarding Oxygen Therapy. This will open the “Add Medications” option.
• Add Oxygen as a medication. Search for the oxygen under the “Full” search field.
• Enter the liter flow in dose; and the mode (NC or Mask, etc.) in amount. Continuous O2 or PRN O2 are entered
in Frequency. Reason can be dyspnea or hypoxia.
At your next in-person visit, Add the Oxygen Pathway via Update POC order type on New Order
• Orders for Oxygen delivery are processed via Parachute (www.parachutehealth.com). Patient demographic
flows from HCHB into Parachute. You will not need a MRN #. Use the patient search feature to find your
patient. If you have any problem ordering through Parachute, contact OPS.

Recertification Process

• 10 days prior to EOE, a recert/dc decision workflow is generated to the CFM.
• When the recert decision is made:
