Urinary Tract Infection


                  Preventing a Catheter-associated Urinary Tract Infection (CAUTI)

               The following is a dialogue between a nurse and a patient after the administration of the
               catheter. Comprehend all the information given in the dialogue to complete the tasks.
                  a.
                    Understand the explanation given by the nurse and identify the detailed information given by the

                    patient to complete on the chart.
                Ns. Ramlan  :  “Good morning, Mr. Haryadi. I’m here to check on your catheter and ask you a few
                                questions about how you’re feeling. I’ll also observe for any signs of infection. This will
                                help us ensure everything is going smoothly and catch any early signs of infection. Is
                                that okay?”
                Mr. Haryadi  :  “Yes, that’s fine. I’m ready.”
                Ns. Ramlan  :  “First, have you noticed any burning sensation around the catheter site or when you
                                feel the urge to urinate?”
                Mr. Haryadi  :  “No, I haven’t felt any burning.”
                Ns. Ramlan  :  “Good. Any pain or discomfort in your lower abdomen, where your bladder is located?”
                Mr. Haryadi  :  “Not really, just a little discomfort from the catheter, but no real pain.”
                Ns. Ramlan  :  “Okay, that’s normal. What about the color or smell of your urine? Have you noticed
                                any cloudiness, blood, or foul odor?”
                Mr. Haryadi  :  “The urine looks normal to me. No blood or unusual smell.”
                Ns. Ramlan  :  “Now, let’s check for any signs of a general infection. Have you felt feverish, had chills,
                                or experienced any unusual fatigue?”
                Mr. Haryadi  :  “No, I’ve felt pretty normal. No chills or fever.”
                Ns. Ramlan  :  “Great,     I’ll   just   take     your    temperature     to     be     sure.
                                Your  temperature  is  normal—98.6°F.  That’s  a  good  sign.  Any  back  pain,  especially
                                around your lower back or the kidney area?”
                Mr. Haryadi  :  “No, no back pain.”
                Ns. Ramlan  :  “Let me take a quick look at the catheter site to examine the area around the catheter
                                insertion.”
                                “All right, there’s no redness, swelling, or discharge, which is excellent. Are you feeling
                                any tenderness or pain around the site?”
                Mr. Haryadi  :  “No, it feels fine, just a little uncomfortable like I mentioned earlier.”
                Ns. Ramlan  :  “That’s good to hear. Everything looks healthy around the insertion point.”
                Mr. Haryadi  :  “Let’s  take  a  look  at  the  urine  output  and  the  drainage  bag.  Are  you  noticing  any
                                decrease in the amount of urine draining or any leakage around the catheter?”
                Ns. Ramlan  :  “No, the drainage seems to be working well. No leaks.”
                Mr. Haryadi  :  “The drainage bag is positioned properly below your bladder level, and the output is
                                good. Everything seems to be in order here.”
                Ns. Ramlan  :  “Have you felt the need to urinate even though the catheter is in place?”
