If you have a concern, follow this path for your resolution:

   1. Go to staff about your concern.

   2. Go to Program Manager about your concern.

   3. Go to Clinical Director for your concern – Rhonda Kiphuth

   4. Go to Patient Advocate for your concern – Alisha Gonzales
   5. Go to Executive Director for your concern. – Kathy Bynum

   6. Go to CEO for your concern by writing down your concern and putting in an
       enveloped Marked “CEO”. Please give your letter to your clinician who will
       give to CEO.

   7. Go to the Arizona Department of Health Services – Residential
      Facilitates Licensing for your concern. 602-364-2639

   8. Go to the Arizona Ombudsman for your concern. 602-364-2536
