Patients with Coronary Heart Disease (CHD) are expected to manage their condition actively, continuously dan most
of the time require various changes in individual and family routines. Patients and their families need to adjust the
disease management and routines to maintain patients wellness. This study aims to explore the challenges
encountered while ensuring self-care among CHD patients, from their families and health care providers’ points of
view. Based on this information, a model will be developed to guide nurses to improve self-care management among
CHD patients. This study is a part of participatory action research, which consists of 4 stages; diagnosis, planning,
implementation, and evaluation. Patients with CHD, their families, nurses, and doctors at the community health
centers and hospitals were interviewed in the first stage of this study.Themes that emerged during the analysis include
1) referral, 2) cooperation, 3) the dynamic of support, and 4) sustainability. Referral refers to steps that patients must
take to get the benefit of insurance and social security. Cooperation means the health care providers, patients, and
family exerts efforts to work closely in maintaining the high health status of the patients. Dynamic support shows the
families’ irregular support for the patients. Lastly, sustainability refers to threats that might endanger self-care due to a
lack of support, and a complicated referral system. The diagnosis stage will inform the planning stage which includes
model development to improve the family’s support for patients’ self-care in managing CHD. Programs will be
created in the planning stage, based on the model developed. The programs then will be informed to the nurses and
ask them to review and evaluate them before it is implemented in the next study.
