              Employment  at  Jack  and  Jill  Children’s  Center  is  contingent  upon  receipt  of  the  completed
              Health Physical (to include a TB test) in which a licensed health care provider indicates fitness
              for duty. A licensed health care provider is contracted to work with Jack and Jill Children’s
              Center to provide physicals for all team members or team members can choose to use their own
              physician. Team members will be required to obtain a health physical upon hiring and every
              two years thereafter at the cost of the team member. Team members who are designated to
              drive the company van must have an annual health physical at an assigned medical facility and
              will be paid for by the organization.

              Team members may be required to submit additional Health Physical/Certifications as needed
              to reasonably accommodate a team member’s disability or to determine eligibility for leave or
              to return to work from any medical/disability leave.

              Health Physicals will be maintained as a part of the team member's file, which is a part of her or
              his personnel record.

              Team members who fail to provide the required Health  Physical as required will be  not be
              allowed to return to work until provided and may be terminated.
