Describe the event to include a short background on what happened and what was known from
the beginning of the incident/event to the end and describe any harm that resulted.
What happened?
• Get multiple perspectives: There is no single best story about what happened.
• Focus on the events that occurred. No blaming during the discussion as this is a fact-finding
mission to improve operations.
• Discuss what events preceded and followed the event.
What was supposed to happen? What did you intend to happen?
• What were your objectives?
• Why did you take this action?
• What were you trying to achieve?
• What were the key assignments?
What was done well? What can we learn about it?
• What are some plausible explanations for the why, when, and where the events happened?
• Look for what went well and lessons about the event.
What was not done well? How can we improve?

• Look for what went wrong and how to improve for the future.
What should we do next time?
