Employee Expectations
          Employees
          	 •	 Shall properly fill out and submit the appropriate paper work (Leave Form), with the necessary medical
          		 documentation certifying their illness/disability/injury, and any information pertaining to the expected
          		 duration of their absence, to management/Human Resources.
          	 •	 Employees are required to continue communication based on a predetermined and agreed upon 		
          		 schedule with Purity Life during the leave, specifically where an end date has not been provided.
          Management/Human Resources
          	 •	 Shall be responsible for accepting, reviewing, and determining the eligibility of all employee requests
          		 for leave benefits.
          	 •	 Shall monitor the employee’s status and liaison with the attending physician for status updates.
          	 •	 Shall ensure ongoing communication with the employee during their time of absence.
          	 •	Be required to work with the physician and employee in identifying accommodation measures
          		 and return-to-work options.
          Performance Assessments, Incentive Plans and Annual Raise Process
          Employees need to be in fulltime employment six (6) consecutive months prior to December 1st to participate in
          the Company’s annual performance reviews and incentive plans and will not be entitled to a merit increase until
          the next annual review process. (applicable for all Leaves; Maternity, Long Term Disability, EI, etc.)
