Leave Policy
Intent
Purity Life has adopted this Leave Policy to assist our full-time permanent employees who are unable to work as
a result of an extended illness, injury, maternity and parental leaves, or disability.
This Policy Shall:
1. provide employee eligibility requirements,
2. discuss employee expectations,
3. determine available benefits, and
4. describe any applicable restrictions.
Guidelines
• The Company shall provide leave to employees who qualify.
• Medical documentation as specified by Purity Life is required to substantiate the need for the leave
and may be requested at various times throughout the leave.
• Purity Life reserves the right to ask for a second opinion by a Company-chosen physician.
• Purity Life shall attempt to find modified duties when possible.
Technology and Access Rights
Directors All electronic hardware and access remain status quo
Managers Electronic hardware remains, but access is limited to email only
(for clarification, please contact HR)
Hardware and access rights cease at time of leave
All Other Staff
Subject to change/exceptions upon Management approval

• Purity Life will ensure all medical information is kept confidential.
• Only those involved in the evaluation of the leave request will view the entire medical file.
• Managers may be required to view the Capabilities Form when modified duties are required.
• Specific diagnoses are not required; only whether or not an employee is able to perform part or all of
their duties.

• Purity Life will ensure all medical information is kept confidential.
