Welcome to the Optimas OE Solution Holdings, LLC(Optimas) beneits
program. It is our pleasure to provide you with this Employee Beneits
Guide for the July 2015 through June 2016 plan year. Its purpose is to
acquaint you with the various beneit plans available to employees of
Optimas. We hope you ind this Guide useful as you weigh your beneit
options.
Optimas offers a comprehensive beneits package aimed to support your
goals of good health and inancial stability. This is a summary of beneits.
Detailed eligibility and beneit descriptions are available on ADP/WFN
portal, or by contacting Human Resources.
Now is your opportunity to enroll in coverage for you and your eligible
dependents. You will not be able to change your medical, dental, and
lexible spending account elections for the July 1, 2015 through June 30,
2016 plan year unless you experience a qualiied life status change. Life
changing events include, but are not limited to:
Marriage
Divorce
Birth or adoption of a child
Changes in your or your dependents’ employment or coverage status
Death
It is your responsibility to log into ADP/WFN portal to request your
change within 31 days of the event. Failure to log into ADP/WFN within
31 days will prohibit you from changing your beneits election(s) until the
next annual enrollment in June for a July 1, 2016 effective date..
