5. You will continue to have the choice between the two MetLife dental
plans with the same coverage as provided by Anixter. Your per-pay-
period-pretax contributions as shown on page 14 of this Guide are 10
percent higher than you contributed to the Anixter plan.
6. Optimas will pay 100 percent of the cost of your basic life and
accidental death and dismemberment (AD&D) coverage. You are
no longer required to pay 20 percent of the cost of coverage and
will automatically be enrolled in this beneit. As you enroll in your
July 2015 beneits, please make sure you designate your life insurance
beneiciary.
7. Short and Long Term Disability coverage will remain unchanged
8. Supplemental life insurance is available to all employees during this
annual enrollment period without answering any medical questions.
The rate per $1,000 of coverage has decreased by 4 to 5 percent for
all age bands!
A. If you previously declined coverage, you may elect up to $100,000
of coverage without answering any medical questions.
B. If you had elected supplemental life coverage through Anixter,
you may increase your current coverage by $100,000 up to a
maximum of $300,000.
C. You must be actively at work on July 1, 2015 for your
supplemental life coverage to be effective.
D. If you do not elect Supplemental Life coverage during this annual
enrollment period and later want to elect coverage, you must
complete an EOI form at your expense and receive approval from
Reliance Standard Life; the only exception is if you have a change
in status; if so, you may elect coverage within 31 days up to the
$300,000 guarantee issue limit without answering any medical
questions
9. EAP services will be provided through HMSA as described on page
20 of this Guide.
10. Optimas will offer an enhanced employer matching contribution
formula to the 401k as a replacement to the PRA contribution.
