Life and Disability Coverage
Life and Accidental Death and Dismemberment (AD&D)
Insurance Coverage
Life insurance is an important part of your inancial security, especially if others
depend on you for support. Accidental Death and Dismemberment (AD&D) insurance
is designed to provide a beneit in the event of accidental death or dismemberment
(loss of limb or paralysis).
F+W provides basic life and AD&D insurance to all eligible employees at no cost to
you. You are responsible for the optional life/AD&D insurance premiums if elected.
X Amount of basic life insurance = $50,000
X AD&D insurance principal sum = $50,000
X Supplemental life insurance is available in $10,000 increments subject
to a maximum of ive times the basic annual earnings or $500,000
whichever is less
X Supplemental AD&D Insurance is available in the following
increments
Z Single—$100,000, $175,000, or $250,000
Z Family—$100,000, $175,000, or $250,000
X Spousal life may be elected in $5,000 increments up to 50 percent of
the insured employee’s supplemental life election
X Dependent child life may be elected in a lump sum of $10,000 or
$20,000 for child(ren) from birth to age 19 or 25, if a full-time
student
Disability Insurance Coverage
The goal of the F+W’s disability insurance plans is to provide you with income
replacement should you become disabled and unable to work due to a non-work-related
illness or injury. The Short Term Disability plan is voluntary and 100 percent employee
paid. The Long Term Disability plan is paid for by F+W.
For details on the cost of any of the voluntary programs, which will be moving to AIG
effective January 1, 2016, please contact Human Resources.
