New Voluntary Coverages
F+W will be offering several new voluntary coverages in 2016. For each new coverage
there is a corresponding beneit summary or brochure with additional information.
Pet Insurance
During open enrollment you have the opportunity to enroll in pet insurance through
United Pet Care. With pet insurance through United Pet Care, you can receive instant
savings of 20 to 50 percent off every veterinary visit, provided your veterinary clinic
participates with United Pet Care. There are no claim forms or deductibles, you simply
just receive a discount or pay a ixed dollar amount for your service. You can purchase
pet insurance for any type of pet – dog, cat, rabbit, ferret, bird, reptile, etc. There are no
age exclusions or pre-existing condition exclusions due to a breed-speciic condition.
United Pet Care offers three different programs: preferred, select, and partner. The
availability of each program depends on your geographic area. Below is a high level
summary of each program.
X Preferred Program
Z 50% savings on ofice visits, annual exams, vaccines
Z 25% savings on procedures and medications
X Select Program
Z $35 ofice visits
Z 20% savings on procedures and medications
X Partner Program
Z 25% savings on ofice visits, annual exams, vaccines
Z 25% savings on in-house medication
The cost of the program depends on how many pets you wish to cover under the plan.
