Dismemberment (AD&D) Insurance
The company automatically provides you this beneit. The beneit is paid
to the beneiciary (or beneiciaries) you designate, in writing. Remember
to designate a beneiciary for your basic life insurance policy for life
insurance and accidental death and dismemberment (AD&D) insurance.
Employee Voluntary Life and AD&D Insurance
You are eligible to purchase voluntary life insurance in increments of
$10,000 up to a maximum beneit of $500,000. You can elect up to
$150,000 in coverage without submitting evidence of insurability (or
proof of good health). The same coverage options exist for voluntary
AD&D insurance. The beneit is paid to the beneiciary (or beneiciaries)
you designate, in writing. Remember to designate a beneiciary for your
voluntary life insurance policy.
Spouse and Dependent Voluntary Life and AD&D
Insurance
If you elect voluntary life coverage for yourself, you can also elect
voluntary life coverage for your spouse in coverage increments of $5,000 Evidence of
up to a maximum beneit of $250,000 or 50% of your elected coverage. Insurability (EOI)
You can elect up to $50,000 in coverage for your spouse without
