Tobacco Surcharge
CareHere charges an additional $23.08 per pay period or $46.15 per pay
period if you (and your spouse) are enrolled in the medical plan and use
tobacco. During open enrollment you will receive the CareHere Employee
Statement of Tobacco Usage form via email. If you are a current tobacco
user, you must register for the tobacco cessation program by December 31,
2018 and must complete the program by May 31, 2019 in order to avoid the
surcharge.
If you are not a tobacco user you must complete the CareHere employee
Statement of Tobacco Usage Form and log onto ADP to designate your
tobacco status—to avoid the tobacco surcharge. If your spouse is enrolled in
the medical plan, then you must also designate his/her tobacco status in ADP.
Employees hired after December 1, 2018 will not be subject to the surcharge
until the 2020 plan year.
