WELLNESS PROGRAM



Details on the 2019 Wellness program will be shared in early January.

2018 Wellness Program

There are currently two components to the wellness program to qualify for the
incentives.


1. Annual Component

† Health risk assessment (HRA) lab & biometrics = $100 in HRA dollars
† Follow-up provider review = $100 in HRA dollars

2. Physical Activity Component

If you take a certain number of steps per day April 1, 2018 – October 31, 2018
(30 week period = average steps per day.) The total number of average steps
earned during the incentive program will correspond to the HRA dollars you
earn. HRA annual dollars are based on average daily steps that convert to a
maximum HRA dollars earned.

Additional 2018 Incentive Reward—Overachievers Club

CareHere recognizes those who are active and healthy and want to encourage
those less active to get moving. We will be rewarding the active individuals
(employee and/or spouse) who walk an average of 10,000 or more steps per
day during the 30 week period.
