Life and AD&D Insurance



EnTrans provides both life insurance and accidental death and
dismemberment (AD&D) insurance at no cost to you. These coverages
are provided through Aetna.


Life insurance provides inancial security for the people who depend on
you. Your beneiciaries will receive a lump sum payment if you die while
employed by EnTrans. The amount of the beneit will be a lat
beneit amount of $50,000.


Accidental death and dismemberment (AD&D) insurance provides
payment to you or your beneiciaries if you lose a limb or die in an

accident. Your AD&D beneit will be a lat beneit amount of $50,000,
and the amount of your beneit will depend on the severity of your
injuries as a result of an accident.

Voluntary Life Insurance



EnTrans provides its employees an opportunity to supplement their
company paid life insurance with a voluntary term life insurance program
offered by Aetna. Employees will be able to purchase voluntary term life
insurance for themselves, their spouses, and their children.


Employees can elect coverage for themselves in increments from one to
ive times their base annual earnings, up to a maximum of $750,000.

Voluntary Life Rates
During an employee’s initial enrollment opportunity, they can elect
voluntary term life coverage for themselves up to $300,000 without Employee Age Employee Rate
per $1,000
