Accident—Alac



Individual accident insurance is a way to stay ahead of the medical and out-
of-pocket expenses that add up quickly after an accidental injury—not just

for emergency treatment, hospital stays and medical exams, but for other
expenses you may face, such as transportation and lodging needs.

When you have an accident, Alac sends a cash beneit directly to you and

you decide the best way to spend your beneit. It’s as simple as that. The
following chart highlights just some of the cash payments available to you in
this program.

Voluntary Accident Benefit Summary


Initial Care Beneit
Emergency room $200 + $100 for 24 hour observation
Urgent care/primary care $125 ($75 for child)

Chiropractor Initial care beneit is $125 ($75 for child) regardless
of which provider is seen; must be within 72 hours of
accident
Hospital beneits
Hospital admission beneit $1,000
Hospital daily beneit $200 (up to 365 days per accident)
ICU daily beneit $400 (up to 30 days per accident)
Accidental Death
