Voluntary Spouse/Dependent Life Insurance
Voluntary Spouse Life
EnTrans employees can elect term life insurance coverage for their spouse. You can purchase coverage in
increments of $10,000 up to a maximum of $100,000. Spousal coverage cannot exceed 50 percent of the
employee beneit and employees’ must select coverage for themselves in order to also elect for their spouses.
Guarantee issue for spouses is $30,000. The cost per each $1,000 of coverage is a lat $0.147 per month. For any
coverage above the guarantee issue amount or for any increases to your currently elected coverage, you will be
required to provide evidence of insurability.
Voluntary Dependent Life
Dependent child life coverage is available in $5,000 increments, up to a $10,000 maximum beneit. Dependents
are eligible from live birth through age 19 (25 for students). The cost for dependent child coverage is either $1.00
per month ($5K beneit) or $2.00 per month ($10K beneit), regardless of number of children.
