Designing of the Data Collection Tool

With the assistance of the World Health Organization (WHO) Sudan, I was able to design the
application mainly in Arabic. The checklist covers all the indicators as designed in the hardcopy

of checklist deigned for supervisors of the Federal EPI and WHO independent monitors. During

the campaign there were two layers of monitoring from both WHO and Federal EPI.
The following checklist was designed for the Federal EPI
