The Oak Creek-Franklin Joint School District Health and Wellness Center
powered by EverSide provides services to employees, spouses, dependents
(ages 2 years of age and older) and retirees covered by the District’s Health
Plan.
Center Hours:
The Health & Wellness Center is open 5 days a week!
Monday - Thursday: 7:00 AM to 5:00 PM
Friday: 7:00 AM to 11:00 AM

Center Location:
The Health & Wellness Center is located inside of the 9th Grade Center (8640 S. Howell Avenue)
on the Oak Creek High School campus. Please enter through Door #11, which faces Howell
Avenue and is the closest door to the intersection of Howell Avenue and Puetz Road. There are
designated parking spots for the Health & Wellness Center directly across from Door #11.

• Employees enrolled on the District’s Medical Plan
• Retirees enrolled on the District’s Medical Plan
• Spouses enrolled on the District’s Medical Plan
• Children 2 years+ enrolled on the District’s Medical Plan
