If you are covered under the Oak Creek-Franklin Joint Wisconsin (OHOW)
School District Health Plan, this service is 100% covered
and you have no patient liability The District has entered into an agreement with OHOW
to allow you to have your orthopedic services provided
by one of their physicians at no cost to you. In order to

identify you as a member covered by our health benefits
you must first contact the Human Resources Department

and let us know you will need to schedule an
appointment to begin the process that may or may not

lead to surgery.

X-Ray services for our covered
members
