This is to inform all ACI employees that if you DO NOT have any changes for your current insurance plan, you are NOT
REQUIRED to fill out the form.
The new enrolment form is required who needs to apply for any changes to previous enrollment.

The application for enrollment (previously declined, addition of dependent(s),change of plan) need to be submitted
(
to me either through E-mail or by hand no later than W Wednesday 7/15/2016.

If you decide to add your dependent(s), additional premium ( (due to adding dependent(s)) will be at your own cost.
Please make a note that insurance company Does Not Allow to add yourself and any dependent(s) after this
enrollment date or after 8/1/2016.

Also, see below the renewal dates:
Health: United Health Care 8/1/2016
Dental: Delta Dental 8/1/2016
Vision: Security Life 1/1/2017

The renewal date is considered as an Open Enrollment date in which you could change the plan(s) and add
dependent(s).
If you would like to review the different plans and/or add dependents, please email your request to info@egins.com and
chris@angelescontractor.com or call 714 533 7089 and ask for Jay.
You must submit the application(s) before the renewal date(s).
