                    Malignancy diseases which are confirmed on Histopathological Report
                    Encephalitis(Viral)

                    Neuro Surgery
                    Total Replacement of Joints
                    Liver Disorder associated with complication of Cirrhosis of Liver
                    Grievous injury which include
                    Multiple fracture of long bone,
                    Head-injury leading to Unconsciousness,
                    Burns of more than 40%,
                    Injury requiring artificial ventilator support plus Vertebral Column Injury



                   3. New Born Baby covered from day one within family floater Sum-Insured applicable to the
                       Employee.

               Other Benefits

               Ambulance charges payable upto a maximum amount of Rs. 1000 per claim.

               Other Terms and Conditions:
                   1. Quote is valid for the given set of data, any change in information will require revised premium.
                   2. Mid term increse in sum insured due to change in level of employee (promotion) is allowed but
                       in case of claims it will not be applicable.
                   3. Quote is valid for 30 days from the date of quote issue or policy start date whichever is earlier.
                       Quote is laible to change with change in information.
                   4. If dependents are to be covered under family structure, then the same needs to be declared at
                       the inception of the policy. Mid term inclusion is allowed only for child on birth and spouse after
                       marriage falling under policy period.
                   5. Psychaitric ailmets/treatments are not covered under the policy.
                   6. All additions/deletions will be done on a pro-rata basis uless otherwise agreed.
                   7. The insured must inform of new additions/deletions within a reasonable time but not later than
                       30 days from the date of new joining of employee or separation of an employee from
                       organisation.
                   8. Domiciliary hospitalisation is specifically excluded.
                   9. Treatement related to genetic disorder is not covered.
                   10. Individuals cannot be covered as an employee and dependent under the same policy nor may
                       children or dependents be covered by both under the same policy.
                   11. List of hospitals where cashless facility can be availed is also available on our website. The
                       company reserves the right to include/exclude any hospital from the list. However if an insured
