MEDICLAIM & INSURANCE POLICY
OBJECTIVE:
The objective of extending high health care coverage to all the employees, the Company provides a
Family Health Insurance Cover and Group Personal Accident Insurance Cover to all the Employees across
the Group.
APPLICABILITY:
All employees of the respective Company who are outside the purview of ESI Act are covered under this
policy.
Mediclaim policy detail
All employees of Indovision are Ensured under Mediclaim policy, each employee is insured for 2 lakh INR

services of any network hospital spread across the country
1. In Case of Hospitalization at network Hospital, the policy holder is required to intimate the TPA
through fax and Courier by sending Hospitalization form as per format prior to admission.
You have to follow the process for Planned Hospitalization
Policy holder intimate TPA 3-4 days prior to admission and then
TPA issues pre-authorization subject to policy term and conditions
Policy holder approaches NWH with I D Card and pre authorization
Cass less treatment extended to policy holder for covered as the agreement
And at the time of discharge, policy holder sings in the claim forms, relevant bills and
leaves all the documents with NWA.
2. In case of Emergency Hospitalization
Policy holder approach the NWA and submit I D Card within 24 hrs, of admission
TPA issues pre-authorization
Cass less treatment extended to policy holder for covered as the agreement
And at the time of discharge, policy holder sings in the claim forms, relevant bills
and leaves all the documents with NWA
