Safety, Health and Environmental Requirements Handbook and an orientation
on its contents. I will read and abide by all of the requirements outlined in the
handbook, all local, state and federal laws and regulations, and any additional
safety and health rules related to my work.

I understand that any violation of these requirements can result in my or my
employer's removal from the project and termination of the contract.
