SiteͲSpecific Hazard Training Ͳ As mandated by 30 CFR §46.11 for surface mines
and §48.11 for underground mines, all contractor personnel must receive siteͲ
specific hazard training before becoming exposed to the hazards. It is the
responsibility of all contractors to ensure that their personnel have received such
training. If the Contractor conducts the siteͲspecific hazard training,
documentation confirming that this training has been conducted will be
provided to the Vulcan Representative. Such documentation must include, but
is not limited to, the written certification pursuant to 30 CFR §46.9(b)(5) Ͳ that
such training has been performed. The rules and procedures provided in this
training will be complied with at all times.
Restricted Access Ͳ Contractor personnel are authorized to access only those
areas of Vulcan property necessary to perform their work activities. All
contractors will take steps necessary to prevent their employees from entering
unauthorized areas. If entry is deemed necessary, the Contractor Representative
Ͳ unless it is an emergency Ͳ will first request authorization from their Vulcan
Representative.
Accident Reporting and Investigation Ͳ All contractors will report any injury,
serious accidents and environmental incidents to their Vulcan Representative as
soon as possible. All contractors are expected to comply with all federal and state
reporting obligations including, but not limited to, those set out in 30 CFR Part
50 and 29 CFR Part 1904. The contractor will provide copies of investigative
reports, corrective actions, as well as any materials submitted by contractor to
MSHA or OSHA.
A copy of the Contractor's 7000Ͳ1 Form, and OSHA 101 (or employer's first report
of injury) and updated OSHA 300 Log (or equivalent if OSHA changes the
recordkeeping rules) will be provided to your Vulcan Representative following
each MSHA or OSHA recordable injury or illness. For environmental incidents
verbal notification, including the type of incident, a description of the events
leading to the incident and corrective actions taken, to their Vulcan
representative is required.
Each contractor is responsible for investigating its own accidents and taking
appropriate corrective action. The contractor must provide to their Vulcan
Representative a copy of an investigative report and the corrective action plan.
