2-202   Reporting Procedures for Incidents Requiring Medical
                                      Attention
                                      A.  First Aid Only – If medical attention is not required, the
                                         supervisor completes the incident notification form.  Both the
                                         employee and the supervisor should sign the form.
                                      B.  Medical Referrals to a Physician – a supervisor or designee
                                         must take the injured employee to the Company's designated
                                         medical facility. Workplace injuries must be treated by
                                         Company-assigned doctors in order to ensure complete
                                         coverage for medical treatment rendered.
                                      C.  The doctor will issue a statement of each visit, which the
                                         employee must give to their supervisor upon return to work.
                                         This statement will include the date and time of next
                                         appointment and will state whether there are any work
                                         restrictions.
                                      D.  When no further treatment is required, the doctor will indicate
                                         "discharge from treatment" and will give the employee a full
                                         release, with no restrictions. A copy must be given to the
                                         supervisor.
                                      E.  If the employee is given medication that may impair the ability
                                         to safely perform work, the employee must notify his or her
                                         supervisor.

                               2-203   Violating the accident reporting policy is cause for disciplinary
                                      action up to and including termination. Termination may be
                                      administered on a first offense.

                               ADVERSE WEATHER CONDITIONS (2-300)

                               Lightning

                               2-300   Stay inside a metal structure, building or vehicle during a lightning
                                      storm unless it is absolutely necessary to leave. Operators of
                                      tracked vehicles should evacuate and seek safe shelter.

                               2-301   Do not stand under any shelter with feet on the ground and
                                      another part of the body leaning against metal walls or objects..

                               2-302   Stay away from trees unless the area is heavily wooded, in which
                                      case it is fairly safe.  Seek low ground.

                               2-303   Stay away from catwalks, fences and metal pipelines.

                               2-304   Do not load holes for blasting, operate drills, do electrical work,
                                      operate electrically powered hand tools or use long tools, such as
                                      crowbars or extension ladders, while lightning storms are in
                                      progress.

                               2-305   If no safe area is available, and you feel static electricity in the air,
                                      crouch down and lower your head, but do not put your hands on
                                      the ground.
