Provided by Reliance Standard, this valuable income protection

program is available to employees working 25 or more hours per week. This
benefit is fully paid by Walter Robbs for employee coverage. The benefit amount

for Basic Life/AD&D is two times employee earnings up to a maximum of
$200,000. Guaranteed issue amount is $200,000. As you age, reductions occur.
The first reduction occurs at age 65. This benefit terminates at retirement.

You may purchase Dependent Life Coverage for an affordable cost of $0.30 per
$1,000, regardless of the number of dependents you cover. Benefit amounts are
as follows:

 Spouse benefit – $10,000
 Child(ren)

o 14 days to 6 months – $1,000
o Child 6 months up to age 20 (age 26 if full-time student) –
$5,000
