Voluntary Term Life
Group# 151394
Provided by Reliance Standard. You may elect to purchase valuable
additional term life coverage at competitive group rates for you, your eligible
spouse and dependent children.
Employee: Increments of $10,000 up to the lesser of five times annual
salary or $500,000. Employee guaranteed issue amount is $80,000 without
providing proof of good health, if enrolled when first eligible. If you are a
late entrant, all amounts of coverage are subject to full medical
underwriting.
Spouse: Available in increments of $5,000 to a maximum of $500,000 (not
to exceed 5 times earnings). Spouse’s amount may not exceed 100% of
employee amount. Spouse guaranteed issue amount is $15,000 without
providing proof of good health, if enrolled when employee is first eligible.
Child: $1,000 for age 14 days to 6 months; increments of $2,000 to $10,000
for age 6 months up to age 20 (up to age 26 if full-time student). All child
amounts are guaranteed issue.
Rates are age-banded and increase in 5 year age.*
As you age, reductions occur; first reduction occurs at age 65. Insured must be
under the age of 75 at time of enrollment.
All premium rates are age-banded based on the age at last birthday. They will
change on the Policy anniversary date coinciding with or next following the
insured's last birthday.
