Flexible Spending Account
Provided by Stanley Benefits
Employees can contribute a maximum of $2,500, to be used for qualified
health expenses such as medical copays, deductibles, dental
and vision expenses.
Employees enrolled in the Health Savings Account compatible plan are
allowed use of FSA funds for expenses related to any benefits excluding
medical.
Employees can contribute a maximum of $5,000 for Dependent Care.
Debit cards will be provided to all enrolled employees at no cost.
Employees are responsible for the cost of additional cards for
dependents.
Fulltime Allegacy employees that do not have Allegacy Health plan cover-
age are eligible to receive additional contributions into their FSA, up to $500,
if they participate in the Allhelath Wellness program. You will be able to earn
$125 FSA credit per quarter and this will be disbursed in January 2018.
Additional Points
Employees are eligible for all benefits on the first day of the month
following 1 month of full-time employment.
Late entrants may be subject to waiting periods for any of the
enclosed benefits, except in cases of qualifying events.
Domestic partners (same or opposite sex) are eligible for medical, dental,
vision benefits. Basic and Voluntary Dependent Life, Whole Life & Allstate
Programs.
