Supplemental Protection Program

Available through Allstate

 These benefits are voluntary and 100% employee paid through payroll
deductions
 These supplemental policies are available to help with the costs of every
day living that may accumulate in the event of a hospitalization, a major
medical diagnosis or chronic disease or medical condition
 Payments are made directly to the policy holder instead of the insurance
company
 Policies are portable and may be taken with you if you leave employment
with Allegacy

 Hospitalization benefits
 Initial diagnosis benefits (optional Initial Diagnosis Benefit Rider available)
 Specified-Disease benefit rider optional
 Continuing Care benefits
 Ambulance, transportation & lodging benefits
 Policy is guaranteed renewable for life
 Portable, with no change in coverage or cost after one month’s payment
on payroll deduction
 Benefits paid to you, regardless of any other insurance you may have

Group Critical Illness Insurance
 Major Critical Illness benefit paid in one lump sum in the event of:
 - Heart Attack
- Stroke
- End-Stage Renal Failure
- Coma
- Paralysis
- Major Human Organ Transplant
 Subsequent Critical Illness benefit for covered conditions in the event of a
recurrence of the same critical illness or an occurrence of a different critical illness
event
 Coronary Artery Bypass Graft Surgery benefit
 Coverage for 29 other specified diseases also included
