-Dislocation or fracture

-Hospital confinement

-Intensive care

-Ambulance

-Accident physician treatment

-Emergency room services

 Coverage is guaranteed issue

 Coverage for employee &/or dependents

 Benefits paid directly to you

 Outpatient physician’s benefit when treatment is received out-
side a hospital setting for any reason, up to 2 visits per year for
each person (4 visits if dependents are covered)
