                                  Basic Life/AD&D Benefits
                                  Provided by UNUM

                                  Basic Life/Accidental Death & Dismemberment

                                  This benefit is 100% paid by Alexander Youth Network

                                     Coverage equal to 1 x your annual base salary for employee  only.
                                      Guaranteed maximum is $100,000. This benefit is available to all full-
                                      time employees working 30+ hours/week.
                                     Conversion & Portability are included.
                                     This is a term life product which reduces as you age; first reduction is
                                      35% at age 70, with the next reduction at 50% of the original amount of
                                      coverage at age 75.



                                  Supplemental Life Program

                                  Provided by UNUM

                                  Supplemental Life
                                     Employee may elect increments of $10,000 to 5x annual salary with a
                                     maximum of $500,000.  Guarantee Issue amount of  $250,000
                                     Your spouse may have coverage,  in $5,000 increments to a maximum
                                     of $500,000, not to exceed 100% of employee’s Optional Life Benefit.
                                     Spouse Guarantee Issue:  $30,000.
                                     Live birth to 14 days $1,000, 14 days to 6 months $1,000 and 6
                                     months to 19, 26 if fulltime student amount of $10,000  not to exceed
                                     100% of employee’s Optional Life Benefit.
                                     Spouse/dependent child coverage is only available if the employee
                                     has purchased supplemental coverage.
                                     Portability is included.
                                     Employee rates are age-banded rates based on the employee’s age
                                     while spouse rates are age-banded rates based on the spouses age.
