Disability Benefits
Provided by UNUM

Short Term Disability
This benefit is 100% paid by ALEXANDER YOUTH NETWORK
 Benefit amount is 60% of employee’s weekly earnings with a
minimum of $25 to a maximum of $1,500 per week.
 Benefits begin on the 8th day of total disability due to a non-work
related accident or sickness.
 12 week maximum benefit duration

Long Term Disability
This benefit is 100% paid by ALEXANDER YOUTH NETWORK
 Benefit amount is 60% of employee’s monthly earnings to a
maximum of $5,000 per month.
 Minimum payout of $100 or 10% of monthly earnings, whichever is
greater
 90 day elimination period.
 For those employees with annual earnings of $40,000 or more, the
definition of disability is your regular occupation to end of benefit
duration.
 For those employees earning less than $40,000, the definition of
disability is your regular occupation for a period of 2 years from date
of disability.

Short Term Disability Buy-Up
 You may purchase additional Short Term Disability with a Health
Screening Rider to cover the remaining 40% of your weekly salary.
