Accident
 Accident protections for yourself and family
 Coverage guaranteed at initial enrollment, no tests as long as
eligibility requirements are met!
 Benefits paid directly to the insurer
 Coverage is completely portable at the same rates

Cancer
 Coverage for the entire family Wellness Screening benefit (see
brochure for details)
 Waiver of premium after 60 days of the total disability due to cancer
as long as the disability lasts
 12/12 Pre-existing condition limitation applies

Critical Illness
 Coverage supplements existing medical benefits to help bridge the
gap of added out of pocket costs due to critical illness.
 100% coverage when you have a heart attack, stroke and major organ
failure, End Stage Renal Failure, just to name a few

Hospital Indemnity
 Pays a specified amount for each day an insured person is confined to
the hospital through a series of optional riders, that can provide
benefits for a range of other medical situations.
 Coverage is for employee and family members
 Coverage cost less than $12 per month

Issue age is 18 & over for all above supplemental coverage.
Accident, Cancer and Critical Illness are pre-taxed.
