Primary Care Clinic Fit-Out at 1010 and 1012 Mattlind Way
Milford, Delaware

Dear Heather,

Bernardon is pleased to submit our proposal for architectural, interior design, and engineering
services for new Primary Care Clinic, to be located at 1010 and 1012 Mattlind Way.

The new fit out will be a complete redesign consisting of approximately 5,000 SF. We
understand Bayhealth Diagnostic Outpatient Center currently occupies 1020 Mattlind Way
and will be vacating this space by the end of the year. The existing primary care clinic will
be moving from a remote existing Milford location to this site. The test fit plan shared by
Bayhealth shows a waiting/reception, exam rooms, treatment room, vitals, soiled and clean
holding, and supporting physician offices.

We have teamed with our engineering consultant Furlow Associates, Inc. for the mechanical,
electrical, plumbing, and fire protection design. Our team understands this is an aggressive
schedule, which Bayhealth plans to use a design build contractor to streamline the bid and
construction process.
