History - Past Medical
It is important when you are doing a medication review.
• Past medical history
• Patient's health status
Purpose of medication
Selection of medication/therapy
Detect drug-disease interactions
• Symptoms/complaints
• Allergies/intolerances
Description of reaction
When reaction occurred
Whether the reaction required treatment
Whether the patient has had a rechallenge
• Renal and hepatic function
• Pregnancy and breastfeeding status. If they are expecting a baby or they are
breastfeeding the medicine can go to the baby as well, not just only the mother.
Past Medical History (PMH) Questions
Tense is important in asking questions.
• What medical conditions do you have or had in the past? (Present or past simple tense)
• What medications are you currently taking? (Present continuous tense)
What are you taking them for?
How long have you taken the medication?
Have you had any side effects from the medication? Has it worsened?
Do you take any medications over-the-counter?
Do you take any herbals?
• Do you have any allergies to any medications?
What was the reaction?
Have you tried it again? What happened?
• Do you have any kidney problems?
• Do you have any liver problems?
• Are you pregnant or breastfeeding?
What trimester are you in?
