        If the first visit, You have to let the patient fill out a patient profile include:
                          •  Name
                          •  Address
                          •  Allergies
                          •  Medications using
                          •  Disease states
                          •  Insurance information





               What does the Pharmacist look at before filling?
                   •  Have I read the prescription correctly? Is that something look-alike sound-alike (LASA)
                       Could I be misreading it due to poor handwriting?  poor handwriting of a doctor that can
                       make you in trouble as well.

                   •  Does the prescription make sense for the patient? For their age? For their medical
               condition?
                   •  Is the dose correct?
