RESOLUTION BY THE PUBLIC SAFETY AND HEALTH COMMITTEE
AUTHORIZING THE PURCHASE OF DEFIBRILLATORS / HEART MONITORS,

AND AUTOPULSE FOR SOUTH SHORE FIRE DEPARTMENT

WHEREAS, the current Autopulses, and Defibrillators/Heart Monitors are
between seven and twelve years old; and

WHEARES, South Shore Fire Department obtained a quote from Physio
Control for a five-year lease/purchase of six Autopulse Lucas Mechanical CPR
units, and ten Defibrillators/Heart Monitors; and

WHEREAS, the South Shore Fire Department is requesting authorization
for the five-year lease/purchase of` Autopulses, and Defibrillators/Heart Monitors
in 2018 at zero percent interest with the first payment being due in 2019; and

WHEREAS, the Fire Department Oversight Board has recommended that
five-year lease/purchase of Autopulses, and Defibrillators/Heart Monitors be
approved by the Village Boards of Sturtevant and Mt. Pleasant; and

NOW THEREFORE, the Village Board of the Village of Sturtevant, Racine
County, Wisconsin does hereby resolve:

1. the Village Board of the Village of Sturtevant, Racine County,
Wisconsin does hereby approve and commit to its share of
funding from Physio Control for a five-year lease/purchase of six
Autopulse Lucas Mechanical CPR units, and ten
Defibrillators/Heart Monitors.

2. The total cost for a five-year lease/purchase of six Autopulse
Lucas Mechanical CPR units, and ten Defibrillators/Heart
Monitors shall not exceed $400,000.

3. The amount owed per year shall be based on the formula set
forth in the Contract for Consolidated Fire/EMS Service.

4. That the funds for said purchase shall be taken from Fund 401
from account 401-57210-810 in the 2019 Budget.

5. That the Village President and Village Clerk authorized to
execute contracts, agreements or other documents necessary
to carry out the intent of this resolution.

Adopted by the Village Board of the Village of Sturtevant, Racine County,
Wisconsin, this 2nd day of July, 2018.
