ii. Daily tim e records. Most entries were handwritten but not initialed
by duly assigned official/personnel. The question is why these
are handwritten when as a practice, em ployees are supposed to
be using the biometrics. Also, there w ere days Mr. Escorpezo
was on OB but the docum ent proving his travel is not the
complete.
iii. MPOR. There were MPORs showing som e w eeks that contained
accomplishments but the DTRs showed that Mr. Escorpezo was
absent.
iv. Leave Ledger Card. The need to reconcile entries following
correct application of crediting leave credits has to be revisited.

3. As to management action, be it ordered that the matter be immediately dealt with
accordingly without prejudice as to whatever sanctions be imposed to Mr.
Escorpezo as per recommendation by your Office.
